The idea
Imagine your pancreas as an engine with a smaller fuel tank than average. Most engines have enough reserve to handle a bad fill-up or an unexpected hill. South Asian beta cells, the cells that produce insulin, run closer to their maximum output at baseline. For years they compensate by working harder, and every test looks normal while they do. That is the trap: the engine revving loudest right before the gauge finally moves. This pattern is shared with other lean-onset populations; what matters clinically is how early it starts and how little warning standard tests give.
