The idea
Kidneys are filters built from units called nephrons, and the supply is set before birth. Developmental factors leave many South Asians with a smaller endowment, a filter with less spare capacity. Every upstream problem in this framework sends damage downstream to it, and once function falls far enough the kidney starts sending damage back, worsening insulin resistance and hardening vessels. A declining trend that still sits inside the normal range is the earliest visible warning, which is exactly why single normal values reassure falsely.
