> Neuroaspergillosis carries near-universal mortality in immunocompromised hosts. Survival here required (a) recognition of chronic otitis media as a fungal portal in an SLE patient, (b) escalation from aspiration to radical excision when initial antifungal-plus-aspiration failed, and (c) prolonged step-down to oral posaconazole. Two months out the patient remained well — a meaningful proof-of-concept for aggressive combined therapy.
