Adipose Biopsy M1/M2 Macrophage Ratio in 322 South Asians — Jaipur
Key finding
Diabetic South Asians had adipocyte HYPERTROPHY in both visceral AND femoral fat depots. The M1/M2 macrophage ratio was significantly elevated in subcutaneous fat (p=0.006), indicating a pro-inflammatory shift in the immune cells infiltrating adipose tissue. Strong correlations were found between MetS score, HOMA-IR, visceral adipocyte SIZE, liver fat fraction, and macrophage ratio (all p<0.001). The critical finding: once you adjust for adipocyte SIZE, the correlation between fat MASS and insulin resistance DISAPPEARS. It is not how much fat you carry — it is how sick each individual fat cell is.
