Porcelain Left Atrium with Giant Calcified Thrombus in Long-Standing Rheumatic Mitral Stenosis

Key finding

A 65-mm LA filled with a layered thrombus (outermost layer calcified and adherent to the LA wall) is the structural endpoint of decades of untreated rheumatic mitral disease. This is not a Western lesion. It is a footprint of late-presentation RHD endemic to SA populations. The intra-operative finding of a thrombus in various stages of organization with calcified outer adhesion to the LA wall demonstrates the layered chronic damage that only late presentation can produce.
